WAIVER – We take photos and videos for promotional use. I grant Mission Accomplished (MA) my permission to photograph, videotape, and/or audiotape myself and those listed in my care during activities with MA. These photographs, video, and/or audiotape will remain the property of MA and may be used for advertising and/or promotional marketing. I hereby waive and release any rights to compensation for or ownership of, such images and/or recordings.

LIABILITY RELEASE – I accept and assume any risk of harm, injury, loss, damage or detriment which may occur as a result of participation in this activity and/or the provision of any services by Mission Accomplished. I release and fully indemnify and hold harmless Mission Accomplished, the directors, officers, agents, employees and representatives thereof, from and against any and all liability, harm, costs or damages, direct or indirect and in whatever form or amount accrued, which might arise from me and my minor children's involvement or participation in any Mission Accomplished activities or services. I further provide my full consent for Mission Accomplished to seek emergency treatment for me and/or my minor children, as deemed appropriate or necessary by Mission Accomplished staff in their sole judgment.

Equipment and Property: I agree to use any equipment, tools, or facilities provided by Mission Accomplished during my volunteer activities with care and diligence. I understand that I am responsible for any damage or loss caused to such equipment, tools, or facilities due to my negligence or misconduct.

I affirm that the facts set forth in this application are true and complete and I fully understand all that is required of me. I also understand that if I am accepted as a volunteer, any false statements, omissions, or other misrepresentations made by me on this application may result in my immediate dismissal. I hereby agree to serve any client who is assigned regardless of race, sex, creed or national origin.
